CHECK LIST FOR ITEMS REQUIRED WHEN SENDING A CASE
POSTERIOR CASES
Final impression in PVS, not poured or an intraoral scan of the prepared tooth
Triple tray or quadrant scan is acceptable for one and two tooth cases
A PVS or wax occlusion record of the preps to opposing (Preferred for scanned cases as well)
Opposing cast, digital scan or impression of opposing
Any relevant photos showing shade tabs with their numbers visible (optional, your preference)
Completed and signed prescription with shade indicated
ANTERIOR CASES
Full arch final impression in PVS, not poured or intra oral scan.
Study cast, digital scan or impression of provisional restorations
Face-bow record to the provisional restorations not the prepared teeth. (Prefer the Kois Facial Plane Analyzer)
Occlusion record of provisional to the opposing (Optional item when less than 4 teeth are treated)
Occlusion record of prepared teeth to the opposing (PVS preferred even for digital scans)
Opposing cast, intra oral scan or impression of opposing
Any relevant photos with shade tabs with numbers visible (a custom shade appointment can be scheduled for local patients)
Partial coverage (veneers) restorations require a photo of the prepared teeth with a shade guide visible (Classic Vita Shades)
Completed and signed prescription with shade information and any specific patient requests/changes to the provisional indicated
IMPLANT CASES
Full arch final impression in PVS with the transfer copings in place, not poured or an intra oral scan with the appropriate scan body. (PVS impressions preferred)
The size and brand of implant system indicated on the prescription, include the implant analogs where possible.
Study cast, digital scan or impression of provisional restorations
Face-bow record to the provisional restorations not the implant sites/prepared teeth. (Prefer the Kois Facial Plane Analyzer)
Occlusion record of provisional to the opposing, use tall healing caps for edentureless posterior situations. (we prefer PVS even for digital scans)
When using healing caps for occlusion records, please send the parts marked with the appropriate tooth number
Occlusion record of prepared teeth/implant sites to the opposing (PVS preferred even for digital scans)
Opposing cast, intra oral scan or impression of opposing
Any relevant photos with shade tabs with numbers visible (a custom shade appointment can be scheduled for local patients)

Occlusion record of provisional to provisional is required (we prefer PVS/wax even for digital scans)
Occlusion record upper provisional to lower preps at the final vertical dimension. (we prefer PVS/wax even for digital scans)
Occlusion record of upper preps to lower preps at the final vertical dimension (we prefer PVS/wax even for digital scans)
